Church Activity Forms

The following forms may be downloaded and used as needed for church related activities. All forms must be submitted to the church office.

Permission Slip Form

Children and youth participating in church activities require formal parental consent. 

For events held at the church, parents need to submit one permission slip which covers an entire church calendar year (September to September).

For events held at locations away from the church (beach trips, ski trips, etc.), parents must fill out and submit one permission slip prior to each event.

One of the following forms must be used to give the required parental consent.

Room Request Form

Members of Westminster Church, church groups (committees, youth groups, etc.) and outside groups must formally request the use of rooms for activities other than regularly scheduled meetings (i.e., committee meetings, regularly scheduled events, etc.). 

The following forms must be used to request room assignments for a single event, or a recurring series of events (repeated group meetings held daily, weekly, monthly, etc.).

Since requests for meeting space by outside groups is reviewed only on a monthly basis, we recommend that outside groups submit requests at least two calendar months in advance of the event.
